Are Achy Knees an Inevitable Part of Aging?

Somewhere along the way, many of us were handed a script about aging: after 50, the knees go. No more running, no more jumping, no more chasing a tennis ball across the court. Just creaks, aches, and caution.

According to Dr. Abigail Campbell, a sports orthopedic surgeon who co-directs the Women’s Sports Medicine Center at NYU Langone, calls the idea that knee pain is inevitable a myth. Yes, cartilage tends to thin as we age, and hormonal shifts can affect our bones. But that doesn’t sentence you to sore knees.

What the research keeps pointing to is this: knee trouble has less to do with your birthday and more to do with how much you move. A 2025 study drawing on national health data from more than 24,000 people found that those with osteoarthritis tended to sit more and move less. And a 2023 study that followed about 2,600 people for eight years found that those with a history of strength training reported knee pain less often.

So the real risk isn’t getting older. It’s doing less.

Why Your Knees Speak Up

The knee is a hinge, not a ball-and-socket like your hip, so it’s less protected. It also absorbs a huge amount of force with every step. That means weakness or stiffness somewhere else in your body often shows up first as knee pain. Sprain an ankle, favor that leg for a few weeks, and your thigh muscles weaken, changing how your kneecap tracks.

Here’s the encouraging part: most of that is fixable. There’s no evidence that running causes arthritis or makes it worse. Your knees may grumble after a big effort, but grumbling isn’t damage.

Strength is the Answer

Strength training is the best remedy for knee pain, and it’s never too late to start. The goal is to build the muscles around the knee so they carry the load instead of the joint itself, especially the kneecap, which is often the first area affected in women. Three keys:

1. Train the whole leg. Isolated knee exercises aren’t enough. Your glutes, hamstrings, quads, and calves all work as a team, so choose moves that use them together: squats, lunges, step-ups, bridges, sit-to-stands, and heel raises.

2. Challenge yourself. Weights that are too light won’t build the support your knees need. Aim for a load where you could do only about two more good reps when you finish a set. Start slowly and keep adding weight as your body adapts and you get more comfortable.

3. Move in every direction. Life isn’t lived in a straight line, and your training shouldn’t be either.

It’s one of the reasons why I developed The Matrix Method, which is a functional full-body workout system that uses the face of an imaginary clock on the floor as a visual guide for multi-directional movement, alignment, and foot placement. I’ll guide you through the process in today’s free workout…
